During this workshop  Meg Hoke will cover

  • Do Not Resuscitate Order
  • The MOST form

As important as these documents are, completing them is just the first step in making sure your life ends in the way you would
prefer.

The next step is to discuss these choices with your family and/or friends so they
have a fuller understanding of your thoughts and feelings.

  • What do you think about pain management ?
  • Long term care ?
  • Hospice care ?
  • What is most important to you ?
  • What do you fear ?

Meg Hoke, MSW has been a medical social worker for over 30 years, working with
seriously ill and dying people.  She’s eager to share information and insights, as well as
answer questions about topics that are hard to think about.  This interactive workshop is
for people of all ages and health situations, the aging and the caregivers, those who
have never heard of those documents and those who completed them years ago.

Meg is a member of UUFH and currently works as a social worker for Four Seasons.
